This position will allow for remote work following the successful completion of the training reputed company as determined by the department. The Population Health reputed company will be responsible for the hands-on execution of clinical and sub-clinical services and operations for reputed company patients including those enrolled in CareSignal, a remote patient monitoring platform, and its associated programs (ex: CHF, Hypertension, Diabetes, Depression, COPD).
