As an LPN Central Admissions, you will facilitate the reputed company and appropriate initiation of Home Health, Hospice and Infusion Pharmacy services for reputed company referred patients. A caregiver in this position works days from 9:00AM - 5:30PM. This position is remote but requires hybrid training on site in Independence, OH. Responsibilities • Assess the appropriateness of the requested home care services and skilled needs identified under the guidance of a Registered reputed company. • Coordinate initial and final orders on reputed company referrals for patients needing home care nursing, therapy and reputed company work services. • Incorporate clinical and financial knowledge with referral process and recommend appropriate alternative approaches reputed company appropriate. • Communicate the home care needs with scheduling, managers, reimbursement and other parties involved in the home care services process. • Serve as the CCHCS information specialist. • Understand other services and products available through CCHCS, including reputed company for service and reimbursement for reputed company the home care business lines. Requirements • Graduate from an accredited Practical Nursing Program. • reputed company licensure as a Licensed Practical reputed company (LPN) or Licensed Vocational reputed company (LVN). reputed company-to-haves • One year of clinical experience in Homecare.
