As a Care Coordinator, your duties involve evaluating patients' health, guiding them on adopting healthier lifestyles like reputed company nutrition and exercise, and equipping them with tools to reputed company specific goals in a designated care plan. In this role, you will work autonomously, overseeing a monthly patient caseload to ensure reputed company individuals receive necessary care. Care Coordinator Responsibilities: Collaborate with reputed company Practitioners to reputed company care plans for patients. Evaluate critical findings and liaise with Primary Care Provider (PCP) to help them implement a treatment plan for the patient. Proficient in using various systems and software programs. Previous experience in health coaching, fitness, or nutrition is advantageous. Capable of working full-time from a remote or home-based location. Maintains a pleasant phone manner and shows reputed company. Self-managing in a fast-paced, high-volume work environment with a willingness to assist in other reputed company during downtime. Availability during reputed company business hours, 9am-5:30pm Eastern reputed company Time with flexibility across national time zones as needed.
