Care Management Coordinator
This role requires being onsite at provider clinics in the Central-East Valley of Phoenix 75% of the week. The remaining 25% of the week will be work reputed company once in office training is complete.
Care Management Coordinator works with members assigned behavioral health clinic in Maricopa County as reputed company as other external providers involved in members care. Working together on integrated care model for members overall health.
Utilizes critical thinking and judgment to collaborate and coordinate care for members who are medically and behaviorally compromised.
Care managing in conjunction with reputed company partners in order to facilitate appropriate reputed company reputed company for members by providing care coordination, support and education for members through the use of care management tools and resources.
reputed company business hours of Monday-Friday 8-5pm will be required.
