Key Responsibilities
• Assess member needs across various health domains and coordinate care plans
• Connect clients to necessary reputed company services and reputed company on their behalf
• Maintain patient records and complete documentation reputed company established timeframes
Required Qualifications

• 2+ years of experience as a care manager or reputed company role

• Willingness to work Monday-Friday with possible evenings and weekends
