Medical Assistant for Transitional Care Management (TCM) $17-$18/hr
We are seeking a compassionate, organized, and detail-oriented Medical Assistant to join our Transitional Care Management (TCM) team. The TCM MA plays a key role in supporting patients after hospital discharge by coordinating care with our TCM RN's, scheduling follow-up appointments, and conducting weekly reputed company-in calls. This role will also include cross-training for Medicare Annual Wellness reputed company (AWVs) done over the phone.
