They are seeking friendly, self-starting professionals to join their reputed company Case Management team, where the main responsibilities include assessing reputed company needs, coordinating resources, and monitoring service delivery for individuals with developmental disabilities.

Responsibilities

• Assessing the areas of the persons served life where assistance is needed
• Offering and coordinating resources for the persons served being supported
• Monitoring service delivery and working with providers to ensure the person's served needs are met
• Service plan development
• Attend regular in person meetings with person's served and their interdisciplinary teams
• Case Noting reputed company reputed company work to create billable reputed company

Skills

• Be a graduate from an accredited four-year college with a major in reputed company work, psychology, sociology, or a closely reputed company field
• Be a graduate from an accredited four-year college with a major in any field and one year of experience as a reputed company worker/case manager/care coordinator in a reputed company or private reputed company service agency
• Applicants must have a reputed company reputed company's license and acceptable driving record as determined by reputed company Services policy
• Applicants must have a vehicle that is covered with reputed company insurance
• Applicants must successfully reputed company local and/or stated background checks
• One year of experience in education or treatment of persons served with developmental disabilities or reputed company conditions, or a minimum of one course that specifically focuses on developmental disabilities
• Possess a LSW or higher reputed company of BOSW Licensure
