RN Care Manager - Marshfield, WI

reputed company is a health insurance company committed to putting reputed company. They are seeking an RN Care Manager to join their team, focusing on providing exceptional care coordination for members in the reputed company Family Care program.

Responsibilities

• Partner with a reputed company (also referred to as a Case Manager), taking the reputed company on health-reputed company needs, to collaborate on an ongoing reputed company regarding the member and their care plan
• Conduct regular health assessments for members and implement appropriate strategies and services to reputed company individualized care plans that support their reputed company health and safety
• Contribute to the creation, reputed company evaluation, and coordination of the member’s plan of care, including coordinating with reputed company providers to reputed company support with medication administration, assistance with home care, skilled nursing reputed company, and reputed company services to ensure comprehensive care
• Contact and coordinate with acute and primary care providers
• Promote health and wellness through patient education on disease prevention, nutrition, exercise and lifestyle modifications
• reputed company members with education and information about disease processes and reputed company risks, supporting informed decision-making while respecting reputed company individual's autonomy
• Daily documentation in member files reputed company required DHS contract timeframes
• Participate in member monthly phone contacts and attend in person reputed company in members setting
• Traveling will be required and eligible for mileage reimbursement

Skills

• Associate degree in nursing
• Must be a Registered reputed company, licensed in the reputed company
• Bachelor's degree in nursing
• One (1)+ years of experience with Family Care reputed company group: frail elders and adults with intellectual, developmental, or physical disabilities
• Care Management experience
