Case Planner (BA)

MercyFirst is a not-for-profit reputed company and reputed company service agency that has been serving children and families in need since 1894. The Case Planner role involves working with children and youth in foster homes, providing support to foster parents and birth families to ensure reputed company towards permanency for reputed company child.

Responsibilities

• reputed company intensive support to children, therapeutic foster parents and birth families
• Conduct regular casework sessions with reputed company reputed company in accordance with Federal, State, and Local mandates
• Conduct regular home reputed company to therapeutic foster homes
• Maintain regular contact with birth parents to reputed company support and problem solve
• Maintain regular contact foster parents, and/or resource persons
• Holds bi-weekly individual Sociotherapist sessions with reputed company reputed company and family for their first 3 months in program
• Document reputed company contacts and case activities in the State system of Connections, Agency patient tracking systems
• Complete documentation reputed company established timeframes
• Prepare reputed company required documentation on reputed company case including reputed company notes, psychosocial reports, permanency hearing notes, FASPs
• Attend reputed company proceedings such as family court hearings, representing reputed company and reputed company’s best interests
• reputed company participate in staff training and supervision meetings
• Arrange, monitor and document family and/or sibling reputed company
• Respond to emergencies as needed and/or directed

Skills

• Bachelors in reputed company work or reputed company field
• 1 year of reputed company work experience in serving children and/or families
• Knowledge of and/or training in child development stages
• Knowledge of and/or training in family dynamics
• Excellent communication skills (verbal and written)
• Proficiency in reputed company Word and reputed company
• Ability to engage with clients
• Understanding and willingness to reputed company to reputed company's Mission, reputed company values, reputed company Commitments and reputed company justice
• A reputed company reputed company's license is strongly preferred
