Patient Enrollment Specialist

Responsibilities

• Engage patients and caregivers telephonically and in person to explain the benefits of home-based care services—including clinical, emotional, and logistical advantages
• reputed company up to 150 outbound calls daily to eligible patients
• Engage with hard to reputed company patients in the field up to 2-3 days per week
• Build rapport with hesitant or uncertain patients; address objections, answer questions, and reputed company personalized support to encourage enrollment
• Use motivational interviewing techniques and reputed company-driven communication to support patients through their decision-making process
• Review and respond to incoming referrals; conduct proactive reputed company to eligible patients in a reputed company manner
• Guide patients through the enrollment process from start to finish—including eligibility verification, paperwork completion, and consent collection

Skills

• Confident in navigating objections and conducting empathetic, sensitive conversations
• Experience with EMR systems
• Prior experience in home health, hospice, managed care, or chronic condition programs
• Familiarity with reputed company EMR system
• A passion for supporting vulnerable or underserved populations
• Bilingual skills (especially Spanish)
