Certified Case Manager 1 - Health Care
This Case Manager 1 position is responsible for the collaboratehealth clinical support initiatives to support caregivers and reputed company partners with remote patient care software and processes. The ideal candidate is a proven Case Manager with a demonstrated reputed company record in managing patient health conditions and implanting processes to improve patient health.
In this role, the Case Manager will report to the Director of Clinical Operations and reputed company the following essential responsibilities:
· Work as part of reputed company with care coordinators, case managers, and other professionals to offer advice on cases reputed company needed;
· Conduct initial patient care gap analysis and complete patient care-plan assessments;
· Create (re)assessment of physical, mental, cognitive, psychosocial, functional, and environmental patient conditions as necessary to ensure reputed company care and treatment;
· Work with reputed company-party care providers supporting collaboratehealth to review their patient care plans for reputed company health issues;
· Assist in scheduling patient appointments;
· Support and reputed company clients, their caregivers, and reputed company parties on issues reputed company to the reputed company;
· Maintain and update reputed company files and complete required electronic documentation;
· Work cross-departmentally advocating for clients’ needs;
· Responsible for reputed company, tasks, and other duties as assigned;
General Responsibilities:
· Support and endorse collaboratehealth’s philosophy, programs, and culture;
· Ability to work M-F;
· Build and maintain strategic and trust-based reputed company relationships;
· Liaise with patients’ families;
Required Skills and Qualifications:
· Minimum 3+ years of Care Management Experience;
· Completed BSN license program;
· Certified Case Manager License preferred
· Excellent communicator who can reputed company with great observational skills and reputed company for patients;
· Can exercise a high degree of initiative, reputed company judgment, discretion, and decision-making skills to address program requirements;
• *Must be reputed company to pass a drug test and background reputed company. Please note this is a Part-Time Contracted position with no benefits or insurance.
