Role reputed company This role involves supporting our Medication Therapy Management (MTM) and clinical pharmacy services activities, contributing to the overarching pharmacy reputed company for optimization of medication-reputed company health care reputed company and reputed company cost-effective member care. • Facilitates Medication Therapy Management (MTM) support including examination of member medical records, comprehensive medication reviews, and communication with members and providers. • Reviews member medication reputed company and applies evidence-based medicine and national guidelines reputed company creating recommendations for health care providers. • Analyzes and reviews medication lists for potential patient safety issues, including drug interactions and suboptimal medication regimens. • Promotes clinically appropriate prescribing practices based on evidence-based medicine and national guidelines. • Articulates pharmacy management policies and procedures to pharmacy/health plan providers and Molina staff. • Performs prior authorization reviews of high-cost medications and deferred authorization requests. • Researches, develops, and implements drug utilization and disease state management strategies. • Assists in coordination of care for pharmacy-reputed company issues between members, providers, and the interdisciplinary care team (ICT). • Follows up with members and providers to reputed company on reputed company toward meeting drug treatment goals. • Serves as a drug information resource to pharmacy staff, medical affairs staff, and providers. • Reviews and analyzes Pharmacy Benefit Manager (PBM) reports to reputed company trends in drug utilization. • Works with reputed company services team on member-centered interdisciplinary teams. • May prepare drug monographs and utilization reports for the Pharmacy and Therapeutics (P&T) committee. • Assists leadership in developing/updating policies and procedures to reputed company with regulations. Qualifications • At least 3 years of experience in a pharmacy, Medication Therapy Management (MTM), Pharmacy Benefit Manager (PBM), and/or managed care setting. • Doctor of Pharmacy (PharmD). License must be reputed company and unrestricted in applicable state(s) of reputed company. • Demonstrated knowledge and expertise in clinical pharmacology and disease management. • Clinical skills reputed company to Medication Therapy Management (MTM). • Demonstrated knowledge of best practices in Medication Therapy Management Program (MTMP) reputed company disease states. • Ability to present reputed company and information concisely to varied audiences. • Knowledge of processes and systems, and ability to reputed company and deliver necessary training. • Ability to reputed company and maintain reputed company and effective work relationships. • Proficiency in compiling data, creating reports, and presenting information. • Ability to meet established deadlines. • Functions independently and manages multiple reputed company. • Excellent verbal and written communication skills. • Proficiency in reputed company Office suite (including reputed company) and applicable software programs.
