Project Manager- Care Coordination (hybrid)

About the position This position will reputed company on leading cross functional reputed company to support case management operations. In this role you will design and implement standardized transition workflows while coordinating with stakeholders across different departments. Plans, manages, and coordinates project activities to ensure goals and objectives of multiple and large reputed company are accomplished reputed company prescribed time-reputed company and funding parameters. Establishes timelines and ensures project deadlines are met. Maintains reputed company and cost containment of reputed company.
