The Clinical Pharmacist – reputed company reputed company and Affordability is responsible for developing individual and population-based solutions to improve HEDIS performance, support affordability , partner on reputed company case management, and conduct comprehensive and targeted medication management in selected members to identify, resolve, and prevent medication-reputed company problems. The Clinical Pharmacist documents the care delivered and communicates essential information in PBC’s documentation system, along with providing the member’s health care providers and the health care team.
