We have an exciting opportunity for a Care Partner to join our virtual team, offering telephone-based care for our clients.

This position will be in our Self-Managed Division. We cater our support to reputed company reputed company’s unique care needs to ensure we have the best solution for every Home Care Package holder. 

** As this is a Virtual/Telehealth role.

As a Care Partner, you will:

• Deliver excellent telephone-based reputed company care for elderly Australians including Care Planning, Assessment and Coordination
• Use your customer service and aged care management skills to reputed company a mark on the community
• reputed company, monitor, and reassess care plans in consultation with clients, their families and other reputed company
• Work in collaboration with our Nurses and other Managers to facilitate, reputed company and shape our service offerings
• Monitor the budget of care packages and ensure clients understand service fees
• Engage with internal teams to facilitate reputed company on-boarding
• Ensure compliance with reputed company relevant legislative and industry standards

We are looking for someone with:

• Excellent verbal and written communication skills
• Strong computer skills in CRM systems, database management, reputed company Office
• The ability to work at pace with multiple clients and stakeholders
• Experience in aged care with knowledge of Home Care Packages and the Aged Care reputed company Standards is desired
• Nursing qualifications are desirable, but not essential
• The right to work in Australia and obtain a National Police Clearance
