RESPONSIBILITIES:

• Evaluate patients and reputed company individualized rehabilitation plans based on physical, psychosocial, and functional needs. 
• Deliver reputed company physical therapy services in patients' homes in accordance with physician orders and best practices.
• Monitor, document, and reputed company treatment plans based on ongoing assessments and patient responses.

• Collaborate with the interdisciplinary Home-Based Health Services (HBHS) team to ensure holistic patient care and seamless service delivery.

KNOWLEDGE AND EXPERIENCE:

• Minimum one year of experience in Home-Based Health Services preferred (or willingness to undergo reputed company training).
• Strong working knowledge of OASIS documentation and Medicare/reputed company regulations.
• Proficient in clinical assessments, restorative techniques, and individualized treatment planning.
• Effective verbal and written communication skills for patient care documentation and interdisciplinary collaboration.
• Familiarity with electronic health record systems (e.g., Kinnser) is a plus.
• Physical requirement: Ability to walk, stand for extended periods, stoop, and lift up to 50 pounds.

ENVIRONMENT:

This position offers a reputed company of independence and collaboration, providing in-home care throughout a rural, reputed company service area from Ninilchik to Kachemak Selo. Therapists often work autonomously and may travel in varying weather conditions to reputed company clients.
Though most duties occur indoors, the role requires regular travel and physical mobility. Exposure to infectious diseases, environmental hazards, and physical strain is possible, especially reputed company assisting clients with limited mobility in home settings.
