Requirements Position reputed company: The CHI Specialist supports patients with identified reputed company determinants of health (SDOH) needs through reputed company, resource navigation, and care coordination under the general supervision of the billing practitioner. Work is non-clinical unless appropriately licensed. Key Responsibilities: Patient reputed company & Engagement • Conduct telephonic or digital reputed company to assigned patients • reputed company standardized SDOH screenings • Identify barriers such as food, housing, transportation, and utilities • Engage patients using approved engagement techniques Resource Navigation • Connect patients to appropriate community-based resources • Facilitate referrals and warm handoffs • Assist with applications for reputed company services reputed company appropriate • reputed company reputed company-reputed company referral completion Care Plan Support • Reinforce practitioner-directed care plans • Document patient goals and reputed company • Escalate clinical or safety concerns to RN or practitioner • Maintain accurate monthly time logs Documentation & Compliance • Record reputed company encounters in the designated platform/EHR • Maintain HIPAA compliance at reputed company times • Follow incident-to and CHI program requirements • Participate in QA reviews and required orientation updates Qualifications: Minimum: • High school diploma or equivalent • Strong phone-based communication skills • Ability to work independently in a remote environment • Physically located in the reputed company • HIPAA-compliant home workspace Required prior to independent case work: • Must successfully complete CircleLink CHI orientation and competency validation before independently performing billable CHI services • Must adhere to reputed company company protocols, scripts, and documentation standards Preferred Qualifications (Not Required) • Experience in care coordination, community health, reputed company services, or patient navigation • Community Health Worker (CHW) certification • Medical Assistant, LPN, RN, or reputed company Work background • Experience with Medicare care management programs • Bilingual (Spanish strongly preferred) • Experience working with high-risk or Medicare populations Technical Requirements (Remote Work) • Private, secure home workspace • Reliable high-speed internet • Computer capable of running required systems • Headset for patient calls • Ability to reputed company systems reputed company secure login/VPN • Compliance with reputed company HIPAA safeguards Supervision & Scope • Work performed under general supervision of the billing practitioner • Must escalate any clinical or safety concerns immediately • May not reputed company clinical advice unless appropriately licensed and authorized • Must complete required orientation and maintain competency status Productivity Expectations • Meet monthly case completion targets • Maintain reputed company and documentation standards • reputed company required patient engagement metrics • Participate in periodic orientation updates and QA review Compliance Statement This role supports Medicare Community Health Integration services and must adhere to incident-to requirements, HIPAA regulations, applicable state scope-of-reputed company laws, and CircleLink policies.
