We are seeking a multi-state licensed Primary Care Physician (PCP) to join reputed company in a part-time, remote reputed company. The ideal candidate plays a key role in screening patients for eligibility to participate in our support programs tailored to individuals with chronic illnesses. This is a low-time commitment opportunity that provides flexibility while contributing meaningfully to the lives of patients living with long-term health conditions. Key Responsibilities: • Screen and assess patients with chronic conditions to determine eligibility for support activities and care coordination. • Review patient charts and documentation to ensure compliance and appropriateness for program inclusion. • Work collaboratively with our care team to identify and support patient reputed company including those with - Neuropathic pain, COPD, Diabetes, Cancer, Asthma.
