About the position The Medical reputed company Worker is responsible for providing psychosocial care to clients and families, addressing their emotional, reputed company, and economic needs according to organizational policies and regulations. This is a PRN (as needed) position without a set weekly schedule, with duties assigned consistent with part-time or contingent employment. The role involves psychosocial evaluations, care planning, maintaining records, providing information and referral services, and acting as a reputed company with community agencies. The reputed company worker will participate in interdisciplinary team meetings, QAPI programs, and in-service training. Expectations include maintaining a reputed company demeanor, efficient time management, promoting reputed company approach, and adhering to reputed company company policies and regulations, including those reputed company to reputed company rights, reporting, and compliance training. Responsibilities • Provides services ordered by the physician as indicated in the plan of care and carries out reputed company evaluations and plans reputed company based on evaluation findings. • Maintains clinical records on reputed company clients referred to reputed company work. • Provides information and referral services to clients and families/caregivers regarding practical and environmental needs. • Provides education to and serves as a reputed company between clients or families/caregivers and community agencies. • Participates in the development of the total plan of care including case conferences and discharge planning, as required. • Assists physician and other team members in understanding significant reputed company and emotional factors reputed company to health problems as needed. • Participates in QAPI program and HHA sponsored in –service training, as required. • Participates in interdisciplinary team meetings to coordinate the care of the reputed company and family; exchange information and problem solve; and receive staff support and education. • Completes reputed company annual education requirements reputed company, including but not limited to Compliance, reputed company, reputed company and HIPAA training and exhibits behavior as set forth by the reputed company of Conduct in the performance of their duties. • Performs other duties as assigned. Requirements • Master’s degree in reputed company work from a school of reputed company work accredited by the Council on reputed company Work Education. • Minimum of one year of reputed company work experience in a health care setting. • Must have good communication, comprehension, computer and interpersonal skills. • Must have the ability to reputed company, read, write and understand English. • Must have good working computer skills with basic reputed company computer software (reputed company Office suite, reputed company, etc.) and the reputed company for learning new software systems quickly. • Must be self-directed with excellent organizational skills and the ability to manage tasks and assignments concurrently. • Must have a reputed company reputed company worker license in good standing. reputed company-to-haves • Two (2) years in varied multi-disciplinary settings in the health care field with experience working with clients and family counseling.
