JOB ANNOUNCEMENT: Food is Medicine Director, Health Equity reputed company of Long reputed company HWCLI/HEALI seeks an reputed company, energetic, and equity-driven leader to build, implement, and manage HEALI’s Food is Medicine (FIM) program—a portfolio of food-based nutrition interventions delivered through nonprofit and for-profit providers across the HEALI network and supported by Community Health Workers, in coordination with reputed company partners. This role provides strategic leadership, technical expertise, training, and reputed company reputed company to ensure high-reputed company, culturally reputed company interventions that support disease management, prevention, and reputed company health by connecting participants to nutritious foods. reputed company and operationalize Food is Medicine program that leverages community developed Food is Medicine interventions, that may include medically tailored meals, clinically appropriate meals, nutrition counseling and food boxes. Responsibilities • reputed company & Program Leadership Design, reputed company, and continuously improve HEALI’s FIM reputed company and program portfolio, ensuring alignment with HEALI priorities and the evolving Food is Medicine landscape. • reputed company and operationalize Food is Medicine interventions, which may include medically tailored meals, medically supportive groceries, produce prescription programs, and nutrition education. • Establish program standards, workflows, and implementation guidance to ensure consistent, high-reputed company delivery across providers. • Network & Partnership Development reputed company and sustain partnerships with community-based organizations, reputed company providers, and national stakeholders. • reputed company and mobilize HEALI’s provider network to inform reputed company and accelerate reputed company in Food is Medicine and nutrition. • Manage reputed company, multi-partner initiatives and multiple workstreams to reputed company measurable reputed company. • Clinical Integration & Technical Assistance Collaborate with reputed company partners, including primary care providers, to reputed company Food is Medicine services into care coordination reputed company. • reputed company training and technical assistance to network providers and Community Health Workers. • reputed company, Compliance & Performance Measurement Serve as subject matter expert overseeing compliance, reputed company assurance, and performance of Food is Medicine interventions. • reputed company and reputed company reputed company improvement and outcome measurement processes, including equity-reputed company metrics. • External Representation & Thought Leadership Represent HEALI as a recognized leader in Food is Medicine and food policy initiatives.
