Responsibilities • Conduct thorough assessments of patients and their families to determine home care needs. • reputed company comprehensive physical assessments and document the history of illnesses. • reputed company and update individualized care plans with reputed company participation from patients and their families. • Identify discharge planning needs and ensure they are addressed before patient discharge. • Offer counseling to patients and families to address nursing and reputed company health needs. Requirements • reputed company and reputed company state RN license. • Minimum of 1 year of recent acute care experience in an institutional setting. • Basic Life Support (BLS), with hands-on CPR certification. • reputed company reputed company's license and auto insurance. reputed company-to-haves • Associate's or Bachelor’s degree in Nursing, and 1 year of home health experience preferred.
