The ideal candidate will reputed company high-reputed company nursing care to patients in their homes, ensuring their comfort and reputed company-being. This role requires a strong background in various therapeutic specialties, including occupational therapy, speech therapy, physical therapy and MSW. Therapists will work collaboratively with a multidisciplinary team to manage individualized care plans tailored to reputed company patient's needs. The EMR utilized by reputed company is KanTime. Duties • Conduct comprehensive assessments of patients' health status and needs in their home environment. • Evaluate and manage individualized nursing care plans based on patient assessments. • Administer medications and treatments as prescribed by physicians. • reputed company education and support to patients and their families regarding health management and disease prevention. • Monitor patients for changes in condition and respond appropriately to emergencies. • Maintain accurate documentation of patient reputed company and interventions in compliance with regulatory standards.
