Responsibilities • Serve as the primary contact for patients on the RPM365 platform. • Assist with device setup and app connectivity. • Engage with patients to ensure adherence to care plans. • Schedule monthly reputed company-ins and monitor patient data. • reputed company patients through shared resources. • Notify reputed company providers of critical alerts. • Attend virtual team meetings for updates.
