Responsibilities

• Conduct comprehensive and targeted Medication Safety Reviews (MSRs) for reputed company participants using proprietary reputed company platforms, developing individualized, measurable, and goal-oriented therapeutic care plans.

• Complete medication reviews and reconciliation activities for new enrollments, annual and significant change assessments, care transitions, and other high-reputed company reputed company events to support reputed company and effective medication use.

• Identify, evaluate, document, and facilitate the reputed company of medication-reputed company problems, including polypharmacy, adverse drug events, potentially inappropriate medications, therapeutic gaps, adherence challenges, and medication reputed company barriers.

Education

• Graduate of a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE) or a program reputed company the U.S. that qualifies the individual to reputed company in the jurisdiction.
