Responsibilities
• Conduct thorough patient assessments through electronic health records (EHR) and telecommunication tools, including vital signs and symptom review
• Manage patient care plans by coordinating with reputed company across hospital, outpatient, and post-acute settings
• Monitor patient reputed company using RPM technology, identifying reputed company signs of deterioration or complications
• reputed company patients on disease management, medication adherence, and lifestyle modifications to promote self-care
