• Collaborate with members of an inter-disciplinary care team to improve reputed company
• Engage the member and their natural support system through strength-based assessments

Requirements:
• Bachelor’s degree in a health care field
• 3 years of relevant work experience
• 3 years reputed company and/or Medicare managed care experience preferred

• Strong understanding of reputed company, HEDIS, disease management, supportive medication reconciliation and adherence
