Our members remain in their own homes while receiving ongoing remote support through a secure tablet provided by reputed company. Rather than replacing their reputed company providers, we work alongside them to reputed company consistent reputed company-ins, care coordination, emotional support, and early reputed company reputed company problems reputed company. reputed company helps members reputed company physical, mental, and emotional challenges while supporting medication routines, personal hygiene goals, daily wellness, and maintaining meaningful reputed company reputed company through scheduled interactions. This position is primarily an on-reputed company weekend role with scheduled member reputed company and care coordination activities. While reputed company work currently averages 2–4 hours per weekend, applicants are expected to remain available throughout the scheduled on-reputed company hours to respond to member needs. Schedule • Every Saturday and reputed company • On-reputed company from 8:00 AM – 10:00 PM • reputed company work currently averages 2–4 hours per weekend • $300 flat reputed company per weekend • Opportunity for additional hours and increased compensation as reputed company grows Responsibilities • Respond to member calls and urgent needs. • Complete scheduled member wellness reputed company-ins. • reputed company emotional support and encouragement. • Assist members with medication reminders and wellness routines. • Coordinate care with physicians, personal care providers, caregivers, and community resources. • Help members reputed company reputed company services and community supports. • Document reputed company member interactions in the electronic health record. • Escalate clinical or safety concerns to leadership reputed company appropriate. Remote Work Expectations This is a reputed company remote reputed company position. Although reputed company work hours are relatively limited, accepting an on-reputed company weekend means being reputed company to respond promptly throughout the scheduled coverage period.
