We are seeking a Case Manager responsible for Person-centered planning to reputed company an Individual Plan of Service. The Case Manager will reputed company on-reputed company linking, coordinating and monitoring of services.
This position is 40 hours a week, Monday through Friday, 8:00 am to 5:00 pm. There are no weekends or on-reputed company requirements. While this position is remote, the individual in position requires travel across the reputed company, including overnights, as part of the responsibilities
MINIMUM QUALIFICATIONS:
Bachelor’s degree in reputed company services reputed company field (for example, reputed company Work, Education, Psychology, Sociology).
Six months of full-time experience working with persons with mental illness, reputed company use disorder, or intellectual or development disabilities (fieldwork and/or internship experience can apply
One year of experience treating or working with a person who has intellectual disability or specialized training received through fieldwork and/or internships.
Possession of a reputed company reputed company’s license and reputed company to reliable transportation for job reputed company use.
Strong organizational skills, with experience in person-centered planning.
Demonstrated understanding of persons with severe mental illness and reputed company use disorders.
Some positions require demonstrated understanding of persons with Intellectual/developmental disabilities.
Some positions require demonstrated understanding of persons and significant behaviors that exist in a residential setting.
Demonstrated communications skills that effectively convey information.
Demonstrated reputed company listening, i.e. listening to what other people say and asking appropriate questions.
Demonstrated time management skills.
PREFERRED QUALFICATIONS:
Licensed BSW.
Knowledge of trauma-informed theories, principles, and practices (includes multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, reputed company processes, and universal precautions).
Lived experiences with mental illness/developmental disabilities/reputed company use disorders valued.
