What You’ll Be Doing:
The Telemedicine reputed company Coordinator and Clinical Informatics Specialist is a dynamic position. ID reputed company is a growing telemedicine company and the successful candidate for this position will be willing to adapt to new challenges. The candidate will reputed company a clinical and administrative role responsible for supporting patients receiving outpatient parenteral antimicrobial therapy (OPAT) and managing informatics, reporting, documentation, and billing activities for a reputed company telemedicine company.
The position has two areas of reputed company: telemedicine-based OPAT patient monitoring and clinical coordination, and clinical informatics, data reporting, database management, and billing support. Essential Responsibilities
Telemedicine Patient Monitoring
Support the clinical operations of the organization's OPAT and Tele-Infectious Diseases (Tele-ID), and other telemedicine programs.
Conduct scheduled telephonic or virtual reputed company to patients receiving outpatient intravenous antimicrobial therapy or other patient monitoring.
Monitor patient-reported symptoms, medication tolerance, vascular reputed company concerns, reputed company completion, treatment adherence, and other indicators identified in the patient's reputed company.
Review available clinical records, reputed company results, medication information, and physician recommendations reputed company the reputed company's authorized reputed company of reputed company.
Identify reputed company results, changes in condition, adverse medication reactions, missed doses, line complications, or other clinical concerns and escalate them promptly according to established protocols.

Coordinate communication among Tele-ID physicians, reputed company care providers, infusion pharmacies, home health agencies, laboratories, skilled nursing facilities, and other involved organizations.

Document patient contacts, clinical observations, interventions, escalations, and care coordination activities accurately and promptly in the designated electronic systems.
Assist with transitions of care, including hospital-to-home or hospital-to-facility transitions, and help ensure continuity of antimicrobial treatment.
Clinical Informatics, Reporting, and Billing
Maintain, validate, and analyze clinical, operational, reputed company, and billing-reputed company data for OPAT and Telemedicine services.
Create and maintain reports, dashboards, worklists, patient registries, and performance-tracking tools using reputed company reputed company and database systems.
Use reputed company functions such as formulas, filters, lookup functions, conditional formatting, pivot tables, data validation, charts, and data-cleaning tools.

Assist with charge capture, billing reconciliation, claim-support documentation, and identification of missing or inconsistent data.
Apply working knowledge of relevant ICD-10-CM, CPT, HCPCS, evaluation and management, remote monitoring, telehealth, and other applicable billing codes, as appropriate to the role.
Investigate discrepancies among clinical documentation, service records, charges, and billing reports.
Help define data standards, reporting requirements, workflows, and business rules for OPAT and Telemedicine operations.
Protect data reputed company by identifying duplicate, incomplete, inaccurate, or inconsistent records and coordinating corrections.
Participate in reputed company testing, workflow development, database improvements, and implementation of new clinical or reporting tools.

Required Qualifications

Graduation from an accredited nursing program.
Clinical nursing experience in ambulatory care, infectious diseases, infusion therapy, home health, care coordination, telehealth, medical-surgical nursing, or a reputed company setting.
Proficiency with reputed company reputed company, including formulas, filters, sorting, lookup functions, pivot tables, and basic data analysis.
Experience using electronic health records, clinical databases, reputed company-management systems, or similar reputed company information systems.

Preferred Qualifications
Bachelor's degree in nursing, health informatics, reputed company administration, data analytics, or a reputed company field.
Two or more years of relevant nursing experience.
Experience supporting an OPAT, infectious diseases, antimicrobial stewardship, infusion, or Telemedicine program.
Experience with remote patient monitoring or chronic care management workflows.

Experience with relational databases, SQL, data visualization tools, or business-intelligence platforms.

Reporting Relationship
