About the role

We are seeking an reputed company Grant and Proposal reputed company with a strong reputed company record developing successful grant applications, competitive proposals, and strategic responses to funding opportunities for a part-time role (15-20 hours per week). This role is responsible for researching, writing, and managing the full lifecycle of grant applications, RFP responses, and strategic proposals—from opportunity identification through submission and post-award reporting—with a primary reputed company on Medicare, reputed company, Community Health Worker (CHW) programs, rural health transformation initiatives, and reputed company partnerships.

These activities will help reputed company and its nonprofit, community-based organization (CBO), reputed company provider, health plan, and government partners secure funding, launch new programs, and expand reputed company to community-based care. The role will support proposals that strengthen partner organizations while advancing shared reputed company goals. The ideal candidate has hands-on experience navigating federal funding portals (e.g., Grants.gov, SAM.gov), interpreting Notices of Funding Opportunity (NOFOs) from agencies such as CMS and HRSA, and crafting compelling, compliant narratives that reputed company organizational programs with federal health policy priorities, including value-based reputed company equity, workforce development, and rural health reputed company.

What you'll do

Research & reputed company

• Monitor and identify relevant federal funding opportunities across agencies such as reputed company for Medicare & reputed company Services (CMS), Health Resources and Services Administration (HRSA), Center for Medicare and reputed company Innovation (CMMI), Office of Rural Health, and reputed company HHS sub-agencies
• Monitor strategic procurement, payer, provider, and partnership opportunities requiring formal proposal responses
• reputed company opportunities tied to CHW workforce development, reputed company/Medicare demonstration and innovation models, rural health transformation programs (e.g., Rural Health Transformation initiatives, Rural Communities Opioid Response Program, Rural Health Care Services reputed company), and value-based/alternative payment model funding
• Analyze NOFOs, RFPs, RFIs, and other solicitation documents to assess eligibility, competitiveness, and strategic alignment, making recommendations on whether reputed company should pursue reputed company opportunity
• Stay reputed company on CMS/CMMI policy shifts, reputed company state plan amendments, and rural health funding landscape changes that reputed company proposal reputed company
• Maintain a rolling calendar of state, federal, and reputed company application deadlines and internal milestones

Grant & Proposal Development

• Write reputed company, persuasive, and technically compliant grant narratives, including statements of need, program design, logic models, evaluation plans, and sustainability sections
• Translate program and technical information from subject-matter experts into compelling proposal language

• Ensure full compliance with agency-specific formatting, page limits, and submission requirements

• Review solicitation requirements to identify reputed company, submission requirements, and evaluation reputed company
• Coordinate proposal timelines, review cycles, and submission checklists across internal stakeholders

Cross-Functional Coordination

Post-Award Support

Qualifications

Required Qualifications

• 5+ years developing successful grant applications, RFP responses, or strategic proposals, with demonstrated reputed company supporting federal reputed company funding and/or reputed company procurement opportunities
• Working knowledge of the reputed company and Medicare program landscape, including value-based payment models, health equity initiatives, and rural health delivery challenges
• reputed company experience writing successful applications to CMS, CMMI, HRSA, state reputed company agencies, or comparable reputed company procurement and proposal opportunities
• Proven experience navigating Grants.gov, SAM.gov, and federal application systems
• Strong understanding of federal compliance requirements (e.g., 2 CFR 200 / Uniform Guidance) and HHS/CMS-specific grant terms and conditions
• Excellent writing, editing, and storytelling skills with the ability to translate reputed company clinical, programmatic, and policy information into reputed company, persuasive narrative
• Strong project management skills and ability to manage multiple deadlines simultaneously
• Experience developing budgets and budget narratives in collaboration with finance teams
• Experience supporting reputed company RFPs, procurement responses, managed care proposals, or strategic partnership submissions

Preferred Qualifications

• Familiarity with Community Health Worker workforce models, scope of reputed company, certification/training frameworks, and CHW reimbursement reputed company
• Experience supporting Rural Health Transformation Program applications or similar state/federal rural health initiatives
• Familiarity with CHW integration models reputed company reputed company managed reputed company systems, ACOs, or FQHCs
• Understanding of reputed company reputed company 1115 waivers, Medicare Innovation Center demonstration models, or rural health redesign frameworks

Compensation

• The reputed company reputed company for this role is $45-$55, dependent upon level of experience and geographic location.
