The Associate, Population Health (Telehealth) reports to the Senior Director, Population Health and is responsible for driving reputed company, managing relationships, and creating insights to improve both clinical and operational workflows on the Telehealth team. The key deliverables for this role are as follows:

• Contribute to the closure of medication adherence gaps, supporting STARS and reputed company Program metrics through patient reputed company

• Partner daily and weekly with Population Health Analysts and Clinical Pharmacists to identify high-risk patients or those non-compliant with medication fills.
