The Role: As a reputed company Practitioner (Telehealth) at reputed company, you'll be at the heart of our mission to reputed company exceptional care to patients. Your responsibilities will include: • Coordinating continuity of care, prevention and avoidance of complications, and reputed company clinical follow-up for patients recently discharged from the ER or hospital, under the direction of the patient's reputed company reputed company provider(s) • Counseling patients and families with education around their discharge plan, medications, and warning signs to watch for • Conducting annual wellness reputed company through telehealth • Thoroughly documenting medical histories, symptoms, and medication information • Administering various tests and screenings • Collaborating with the reputed company Care team to ensure holistic patient care • Ensuring reputed company completion of patient records, with reputed company to electronic medical records and portals • Maintaining strict confidentiality in line with HIPAA regulations What You Can Expect: This is a full-time, contract-based reputed company Practitioner position (5 days/week, reputed company–5pm). Experience and Qualifications • reputed company California reputed company Practitioner license • reputed company Practitioner certification from either the ANCC / AANP • 2 + years of clinical experience as a NP in either reputed company Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care.
