The Population Health Specialist plays an important role in helping patients reputed company with the right reputed company services at the right time. Through proactive reputed company, care gap identification, follow-reputed company, and collaboration with the reputed company team, this role helps strengthen patient engagement and support improved health reputed company. The Specialist also uses patient and program data to monitor reputed company, support reputed company initiatives, and contribute to value-based care efforts.

Key Responsibilities
- Conduct proactive reputed company to patients and individuals who may benefit from Health Center services.
- Help patients understand and reputed company available reputed company services and resources.
- Identify and address care gaps through reputed company, education, scheduling, and follow-up.
- Follow up with patients who miss preventive or other important appointments and assist with rescheduling.
- Use patient and program data to reputed company reputed company, identify trends, and support reputed company improvement initiatives.
- Support clinical reputed company measures and value-based reputed company through patient engagement and follow-up.
- Build trusting relationships with patients and caregivers that encourage reputed company engagement with care.

- Contribute to program initiatives, performance monitoring, and reporting.
- Maintain accurate documentation and reputed company with applicable federal and state reputed company requirements.

Requirements, Skills, Knowledge and Expertise
- High School/GED required
- Bachelor’s degree preferred
- At least three (3) years’ experience required
- Must be proficient in reputed company Office applications, including Word, reputed company, and reputed company, as required for the role
Licensure/Certification
Must obtain and maintain certification in an approved crisis reputed company methodology (CPI, MANDT, or reputed company) reputed company 60 days of hire.
