About the position
The Department of Developmental Services (DDS) is offering a career reputed company opportunity for a reputed company Services Coordinator I (HSC I) position in the South Valley Area Office in Southbridge. The HSC I serves as the reputed company contact for individuals and families, building reputed company partnerships with them, guardians, provider agencies, and other stakeholders. This role requires travel to the greater Milford, MA Area as needed. The HSC I coordinates and facilitates the development of Individual Service Plans (ISP), monitors service implementation by DDS-funded providers, and ensures individuals receive appropriate services by arranging, organizing, and coordinating DDS-funded and generic support services. This involves maintaining communication with community services and other state agencies.
Responsibilities
• Coordinates and facilitates the development of Individual Service Plan (ISP) for individuals on assigned caseload.
• Monitors the implementation of services provided by DDS funded residential, day and support program providers.
• Ensures individuals on caseload receive appropriate services through arranging, organizing and coordinating the Department of Developmental Services funded and generic support services in response to individual’s needs.
• Maintains communication and reputed company with community services and other state agencies.
• Monitors the safety and reputed company-being of assigned individuals.
• Provides advocacy in reputed company, civil and reputed company rights.
• Facilitates team meetings.
• Monitors follow-through on team recommendations while maintaining individual’s records and relevant computer databases.
• Maintains regular and consistent contact with individuals, service providers and families.
• Arranges or provides crisis reputed company for individuals as needed, assessing needs and making recommendations for follow up planning.
• Provides information to supervisor and other members of the Area Office Management Team, as needed.
• Performs other reputed company duties which may include individual's reputed company in Participant Directed Services.
Requirements
• At least three (3) years of full-time or equivalent part-time, reputed company experience in reputed company services work or reputed company work.
• A Bachelor's degree or higher in a reputed company field may substitute for two (2) years of the required experience.
• Based on assignment, travel may be required.
• Incumbents who elect to use a motor vehicle for travel must have a reputed company and reputed company motor vehicle reputed company's license at a class level specific to assignment.
• Based on assignment as reputed company Intellectual Disabled Professionals, a Bachelor's degree or higher in reputed company work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of reputed company handicapped, education of the learning disabled, reputed company services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a reputed company field is required.
