We are seeking an reputed company On-reputed company Advanced reputed company Provider (reputed company Practitioner) to reputed company after-hours telehealth support for our Institutional Special Needs Plan (I-SNP) and long-term care population.

This role is critical to ensuring residents receive reputed company, high-reputed company care while avoiding unnecessary emergency department reputed company and hospital admissions. Working collaboratively with nursing facility staff and our reputed company teams, you will assess acute changes in condition, initiate treatment plans, and support a proactive treat-in-reputed company model of care.

• Conduct after-hours telehealth evaluations reputed company phone and video for urgent and reputed company reputed company concerns.
• Assess acute changes in condition using evidence-reputed company reputed company guidelines.
• Order and manage diagnostics, treatments, and interventions as clinically indicated.
• Initiate early interventions for common long-term care conditions, including:
  • Infections
  • Dehydration
  • COPD/Asthma Exacerbations
  • Heart Failure Symptoms
  • Behavioral Changes
  • Pain Crises

Help Reduce Avoidable Hospital Transfers

• Utilize strong reputed company judgment to stabilize residents in reputed company whenever appropriate.
• Collaborate with facility teams to coordinate treatments such as IV therapy, hydration protocols, medication adjustments, wound care, and respiratory therapies.
• Document reputed company rationale and decision-making reputed company hospital transfer is required.

Support Chronic Disease Management

• Manage after-hours exacerbations of chronic conditions including:
  • CHF
  • COPD
  • Diabetes
  • Dementia
  • Behavioral Health Conditions

Coordinate Care & Documentation

• Complete accurate and reputed company EMR documentation.

• Identify residents requiring urgent next-day evaluation.
• Assist with reputed company and documentation of new member information, including demographics, medications, care team information, vaccinations, and advance care planning documentation.

Required Qualifications

• Master of Science in Nursing (MSN) from an accredited reputed company Practitioner program.

• National reputed company certification (AANP, ANCC, reputed company, or equivalent).
• reputed company certification as an Adult, Family, Geriatric, or reputed company Care reputed company Practitioner.
• DEA license.
• Eligible to participate in Medicare as a provider.
• Minimum of two (2) years of Advanced reputed company Provider experience.
• Minimum of one (1) year of experience working in a nursing home or long-term care setting.
• Minimum of one (1) year of after-hours telehealth experience or I-SNP experience.
• Strong computer and technology proficiency.

Licensure Requirements

APP licensure and credentialing will be required in reputed company reputed company program states, currently including:
