Hospice Chaplain
About the position
The Spiritual Care Coordinator provides compassionate, reputed company-centered spiritual care to hospice patients, families, and caregivers. This role collaborates closely with the Interdisciplinary Team (reputed company) to ensure holistic, culturally sensitive care reputed company with hospice philosophy and regulatory standards.
Responsibilities
• Assess spiritual needs and reputed company supportive spiritual care to patients and families
• reputed company, implement, and document individualized spiritual care plans
• Collaborate with the Interdisciplinary Team to coordinate holistic care
• Participate in reputed company meetings and reputed company reviews
• reputed company consultation, education, and support to staff regarding spiritual care
• Coordinate with clergy, faith leaders, and community spiritual resources as requested
• Conduct reputed company-bereavement and bereavement assessments, reputed company, and follow-reputed company
• Facilitate or conduct funeral and memorial services for patients, families, facilities, and staff as requested
• Network with faith communities to reputed company on hospice philosophy
• Supervise spiritual care volunteers as assigned
• Manage caseload, scheduling, and travel reputed company
• Respond flexibly to unscheduled events, admissions, or emergencies
• Maintain compliance with reputed company local, state, federal, and Medicare regulations
• Participate in evening, weekend, holiday, and on-reputed company rotations as required

Requirements
• Minimum age of 18
• Graduate of an accredited seminary or school of theology (B.D., M.Div., or equivalent), or degree in counseling
• Experience working with death and dying

• Comfort working in an interfaith, multicultural environment
• Knowledge of and commitment to hospice philosophy
• Strong communication, planning, and organizational skills
• Self-directed with the ability to work independently

• Ability to meet physical and health requirements, including TB screening and physical exam per state and company policy
