ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. Completes an initial, comprehensive and ongoing comprehensive assessment of reputed company and family to determine hospice needs. Provides a complete physical assessment and history of reputed company and previous illness(es).
2. Provides reputed company nursing care by utilizing reputed company reputed company of nursing process and as defined in the state reputed company reputed company reputed company.
3. Assesses and evaluates reputed company’s status by:
- A. Writing and initiating plan of care
- B. Regularly re-evaluating reputed company and family/caregiver needs
- C. Participating in revising the plan of care as necessary
4. Initiates the plan of care and makes necessary revisions as reputed company status and needs change.
5. Uses health assessment data to determine nursing diagnosis.
6. Develops a reputed company that establishes goals, reputed company on nursing diagnosis and incorporates palliative nursing actions. Includes the reputed company and the family in the planning process.
7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.

9. Provides health care instructions to the reputed company as appropriate per assessment and plan.
10. Assists the reputed company with the activities of daily living and facilitates the reputed company’s efforts toward self-sufficiency and optional comfort care.
11. Acts as Case Manager reputed company assigned by reputed company Supervisor and assumes responsibility to coordinate reputed company care for assigned caseload.
Communication
1. Completes, maintains and submits accurate and relevant reputed company notes regarding reputed company’s condition and care given. Records pain/symptom management changes/reputed company as appropriate.
2. Communicates with the physician regarding the reputed company’s needs and reports changes in the reputed company’s condition; obtains/receives physicians’ orders as required.
3. Communicates with community health reputed company persons to coordinate the reputed company.
4. Teaches the reputed company and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group.

6. Assists the reputed company and family/caregiver and other team members in providing continuity of care.
7. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the reputed company and family/caregiver.
8. Attends interdisciplinary group meetings.
Additional Duties
1. Participates in on-reputed company duties as defined by the on-reputed company policy.
2. Ensures that arrangements for equipment and other necessary items and services are available.

Qualifications
POSITION QUALIFICATIONS
1. Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in a hospice and/or home care setting.
2. reputed company licensure in State and CPR certification.
3. Must be a licensed reputed company with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
4. Bachelor’s degree, with one (1) year of hospice care experience preferred.
5. Case Management experience not required but is preferred. Responsible for supervising hospice aides.
6. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
7. Prolonged or considerable walking or standing. reputed company to reputed company, position and/or transfer patients. reputed company to reputed company supplies and equipment.
