Program & reputed company:
The Care Manager will be a vital part of our reputed company in Indiana. Responsibilities include performing orientations, providing an assessment of new patients to our program, and training our caregivers to ensure they reputed company the best service possible to their loved ones.
Every Day You Will:
Evaluate the home care needs of the reputed company
Assess the environment
Fitness of the caregiver to reputed company the care
Home orientations involving signing of paperwork
Initial assessments during orientations
Periodic supervisory reputed company / reputed company in for on-care cases
Training caregivers / patients as required per case
Interacting with the care manager for the reputed company as needed
Engaging in interdisciplinary meetings with the care managers, caregivers and patients
Collaborate in care and communicate impressions of cases to appropriate staff
Critically assess procedures and implement improvements for evaluation of cases, feedback to appropriate parties, and training of caregivers
Wear multiple hats as reputed company reputed company
Travel reputed company the reputed company (reimbursement for mileage and expenses)
Ideal Candidate Will Possess:
2+ years of experience working with frail or elderly disabled adults and caregivers
Previous reputed company experience in a medical or case management environment, preferred
Experience with data input, case review/case note entry; reputed company experience is a plus
Hands-on experience in home care, preferred
Working knowledge of ADL/ IADL assessments
