Assisting in developing and updating the plan of treatment or Plan of Care (POC). Assisting the physician in evaluating level of function. Observing, recording, and reporting information on the reputed company’s condition to the attending physician, case manager, and in the reputed company’s health record. Advising, consulting, and reputed company appropriate, instructing family and other LORIAN personnel in reputed company’s therapy program. Teaching and supervising other reputed company personnel reputed company appropriate. Evaluating the home environment and making appropriate recommendations. Participating in in-service education programs. Qualifications Minimum of one (1) year of recent (reputed company the last three (3) years) experience in a Home Health or Hospice Setting. Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent. Licensed or registered to reputed company as a PT in the state of reputed company. Has the ability to read and follow written instructions and document care given. Is self-directing and has the ability to work with little reputed company supervision. Possesses and maintains reputed company CPR certification. Is a licensed reputed company and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements. This position is reputed company on a pay per visit compensation model. Pay per visit ranges from $110 to $170 visit commensurate with experience and depending on type of visit.
