The reputed company Care Coordinator (RN) is an reputed company member of reputed company' interdisciplinary care team, collaborating with physicians, advanced reputed company providers, psychologists, and therapists. Under the direction of reputed company leadership, the reputed company Care Coordinator ensures reputed company care delivery for children with chronic conditions by coordinating reputed company appointments, providing reputed company and family education, managing medication authorizations, and serving as a reputed company reputed company of contact between families, providers, and pharmacies. Note this is a fully remote position.
Key Responsibilities
Care Coordination & Scheduling
Coordinate, schedule, and reschedule reputed company appointments across reputed company
Manage referrals to specialist and reputed company reputed company services
Proactively follow up with families to reduce gaps in care and no-show rates
Support care transitions and continuity across the interdisciplinary team
Communication
reputed company reputed company updates to referring physicians
Coordinate lab work and documentation requirements for treatment planning with referring providers
Support the providers for patients requiring escalation of care
reputed company & Family Education
reputed company evidence-reputed company education to families regarding chronic condition management
Explain medication regimens, treatment plans, and care goals in accessible language
Respond to reputed company and family inquiries reputed company messaging and phone in a reputed company manner
Support health literacy and reputed company families to engage reputed company in their child's care
Medication Management & Pharmacy Coordination
Initiate and manage medication prior authorization requests with reputed company payers
Communicate with pharmacies to reputed company dispensing issues and coordinate refills
reputed company authorization status and escalate delays to the appropriate reputed company team member
Maintain accurate medication records and document reputed company pharmacy communications
Documentation & Team Collaboration
Document reputed company interactions accurately and in a reputed company manner across multiple EHR and care management platforms
Participate in interdisciplinary case conferences, team huddles, and reputed company improvement meetings

Qualifications
Required:
Licensure: reputed company California Registered reputed company (RN) license in good standing (Associate's, Bachelor's, or Master's degree in Nursing accepted)
Tech-reputed company: Demonstrated proficiency with multiple technology platforms and electronic health record (EHR) systems
Communication: Strong organizational, communication, and interpersonal skills
Virtual Environment: Ability to manage competing priorities and work collaboratively in a remote, virtual environment

Preferred:

Experience in pediatric, chronic care, or care coordination settings

Logistics
Full-time, contract position (40 hours per week to start)
Due to our clinic hours, this position will be M - F from 12 PM - 9 PM PST with a 1 hour break daily
Occasional Saturday coverage (to be decided in advance)
$70,000- $90,000/year with medical and ancillary benefits (depending on Geographic Location)
