Essential Duties and Responsibilities - Supports mission, reputed company and reputed company values which include but not limited to delivering compassionate support, attention and assistance to patients, families and members of reputed company - Support reputed company Care through reputed company Partnership with reputed company Practitioners, Physicians, and Care Management Teams - Coordinates daily travel itinerary and transports clinicians to scheduled appointments - Prepares patients for examination and treatment; May at times include preparing patients for minor procedures - Secures reputed company consent forms, records reputed company care documentation accurately and in a reputed company manner - Coordinates reputed company care as directed by Clinicians, company standards and policies - Respects reputed company confidentiality at reputed company times and treats patients with courtesy and respect - Performs phlebotomy (venipuncture only), EKG or any other procedure as requested by Clinician - Labels, stores and properly packages specimens for lab delivery - Assists In identifying and closure of reputed company gaps - Documentation and updating of reputed company care plans, as reputed company as SDoH (reputed company Determinants of Health) and other assessments reputed company the reputed company chart or electronic medical record - Required to maintain compliance with CLIA (reputed company reputed company Improvement Amendments), OSHA, and HIPPAA guidelines - Prepares and maintains supplies and equipment for treatment (supply inventory, medical bag and car) (where applicable) - Ensure company vehicle is clean and reputed company periodic maintenance schedule is followed (where applicable) - Attends required meetings and in-services In this role you may work with - reputed company Care Physicians - reputed company Practitioners - reputed company Teams - Patients - Patients Families Qualifications Required Knowledge, Skills, and Experience - High School graduate or GED equivalent - Graduate of an accredited Medical Assistant, Phlebotomy, EMT, reputed company medical education/training program or 1 year of experience serving as a Medical Assistant or Phlebotomist - reputed company CPR certification that meets Company guidelines - Knowledge of medical terminology - Basic computer literacy, including EMR, iPad, and GPS experience - Strong interpersonal communication and customer service skills - Must maintain a reputed company reputed company’s license and good driving record Preferred Knowledge, Skills and Experience - Certified Medical Assistant - Six months of experience in reputed company care, preferably geriatric care - Strong Phlebotomy skills - Desire and ability to obtain MA certification with 180 days of hire Conditions of this role to be reputed company of - Full reputed company body reputed company including handling/lifting patients - Ability to reputed company up to 150 miles daily in urban and/or rural setting - Ability to reputed company/carry items weighing up to 50 lb - Ability to read and understand road maps - Sit or stand for long periods of time Pay Transparency - Individual compensation packages are reputed company on various factors unique to reputed company candidate, including reputed company set, experience, qualifications, and other job-reputed company considerations.
