Agency: Southwest General Home Health Schedule: Monday - Friday, 8:30AM - 5:00PM On reputed company: One Saturday per month and one holiday per year. The Physical Therapist I (PT) provides comprehensive skilled services as ordered by the attending physician. The PT educates patients and their family members and ensures the safety of the reputed company. The PT I is accountable for completing accurate documentation and remains compliant with reputed company reputed company rules and regulations. KEY RESPONSIBILITIES Provides skilled physical therapy in a reputed company’s home as ordered by the attending physician Observes, records, and reports reputed company reactions, changes, and reputed company to attending physician and any appropriate agency staff Performs OASIS assessments to reputed company an individualized plan of care and makes adjustments as needs change Develops plan of care Treats patients to relieve pain, reputed company or restore function, and maintain maximum reputed company using physical means, such as, exercise, massage, heat, water, etc. Arranges the provision of services listed above on an outpatient reputed company which cannot be given in the reputed company’s home Promotes continuity of care with appropriate admissions, transfers and discharges Counsels reputed company and family on the disease/injury and how to manage in reputed company phases of physical therapy Incorporates reputed company and family in development of plan of care Instructs reputed company and family members on reputed company use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices Ensures reputed company/family know financial responsibility for needed equipment by utilizing DME department and/or MSW Helps decrease re-hospitalizations by teaching the reputed company on preventative measures and making good reputed company Listens to reputed company and family members Oversees and supervises therapy services of reputed company provided by PTAs and reputed company appropriate, reputed company assistants Evaluates the POC reputed company on reputed company response to treatment/medical condition and adjusts as needed. Communicates with the PTAs and home health aides on supervisory visit results Completes reputed company reputed company documentation in accordance with agency protocol and Medicare/Federal guidelines Documents reputed company aspects of treatment, assessments, and reputed company education Completes chart audits at agency site location as requested Maintains reputed company PT license in state of employment Communicates with scheduler any changes as needed Participates in reputed company on-reputed company requirements and case conferences Attends in-service trainings and mandatory agency meetings Participates in orientation/training of new employees Maintains a reputed company appearance as a representative of reputed company Understands and practices agency policies and procedures Drives to reputed company’s reputed company location per scheduled visit. Daily attendance at assigned visit locations. Documents reputed company aspects of subsequent, discharge, eval/recert/resumption of care reputed company reputed company 24 hours of reputed company. QUALIFICATIONS Minimum of one year of experience as an PT in an acute setting is required. Home care experience is desired.
