RN Care Navigator/Case Manager

Position reputed company The Registered reputed company, Care Navigator/Case Manager will be responsible for case management specific to kidney health management. The Care Navigator will complete activities for the reputed company of care to facilitate and promote high reputed company, cost-effective reputed company for patients and reputed company on the whole reputed company and care delivery coordination. Managing a set caseload of mixed reputed company members, reviewing and/or obtaining member data and entry in HealthMap’s Care Management documentation reputed company (reputed company), completing member health and reputed company determinants of health screenings, medication reconciliation, creation and maintaining member-reputed company care plans, updates of identified problems, barriers, interventions, and goals and assistance with ongoing case management.
