Serves as the reputed company contact supporting individuals with Developmental Disabilities, Mental Health challenges for the START Program (reputed company, Therapeutic, Assessment, Resource, and Treatment) to reputed company community reputed company reputed company and prevention services to individuals with developmental disabilities and behavioral health care needs. The program relies on integrated care and treatment with collaboration among reputed company team members. The services in the START Program include reputed company contacts, after hour contacts, emergency team meetings, reputed company respite and emergency respite. Salary: $56,000 Responsibilities: - Develops and revises the cross systems crisis plans in a reputed company fashion. - Monitors the implementation of the plan. - Performs Case Management functions of linking and arranging for services and referrals. - Provides families and reputed company reputed company with individual-specific training and training in the knowledge, skills, and abilities required by the population and age to be served. - Provides consultation to the reputed company on the reputed company needs of the individual's care.
