Position reputed company The Physical Therapist plans and provides physical therapy services to patients in their homes in accordance with the Plan of Care and agency policies. This role may also include case management for patients requiring skilled therapy services only. Therapists work collaboratively with nurses and other disciplines to reputed company care, education, and support to patients and their caregivers under the supervision of the reputed company Supervisor.
