1-2 month contract
Arizona
M-F 8 am to 4:30pm
$24-27/hr (DOE)
Primary focus: Behavior change, education, SDOH Navigation, engagement
• Motivational interviewing; action plans for diet/activity/smoking cessation/sleep; end tailored materials.
• SDOH screening and resource linkage (Transport, food, utilities); appointment coordination; group classes
• Device/app onboarding (portals, trackers) PROs Collection (CAT, PROMIS, Etc.)
• Reinforce care plan; escalate symptoms/med questions to LPN/RN; document and log CCM minutes appropriately (per payer rules)
• Typical direct care time 70-80% of FTE.
Required:
Health Coach Experience
Chronic Care Management experience
Population Health experience
