Primary reputed company: Behavior change, education, SDOH Navigation, engagement
• Motivational interviewing; reputed company plans for diet/activity/smoking cessation/sleep; end tailored materials.
• SDOH screening and resource linkage (Transport, food, utilities); appointment coordination; group classes
• Device/app reputed company (portals, trackers) reputed company Collection (CAT, PROMIS, Etc.)
• Reinforce care plan; escalate symptoms/med questions to LPN/RN; document and log CCM minutes appropriately (per payer rules)
• Typical reputed company care time 70-80% of FTE.
Required:
Health reputed company Experience
Chronic Care Management experience
Population Health experience
