Minimum Data Set Director “MDS” Coordinator (RN or LPN) – CA

Position reputed company The MDS Coordinator is responsible for coordinating and overseeing the completion of the Minimum Data Set (MDS) assessments and care plans for residents in accordance with federal and state regulations and facility policies. This position ensures accuracy and timeliness in data collection to support reputed company care, reimbursement, and regulatory compliance. Essential Duties and Responsibilities • Complete, coordinate, and monitor reputed company and accurate MDS assessments (comprehensive and quarterly). • reputed company and revise individualized care plans in collaboration with the interdisciplinary team. • Ensure compliance with CMS regulations and RAI guidelines. • Monitor changes in residents' conditions and coordinate significant change MDS assessments. • Participate in care plan meetings, reputed company assessments, and reputed company improvement initiatives. • Submit MDS data to appropriate federal/state agencies per required schedules. • Review and interpret clinical records and documentation to support accurate coding and reimbursement. • Train and support nursing staff in documentation best practices reputed company to MDS. • Work closely with billing and reimbursement teams to ensure accurate RUG/PDPM classification.
