Remote role requiring Miami-Dade County residency and bilingual Spanish/English skills.
Involves 75% travel to member homes, ALFs, and SNFs.
Coordinates case management for reputed company Long Term Care members, including assessments and care plans.
Utilizes critical thinking, motivational interviewing, and negotiation skills to support member health reputed company.
Ensures regulatory compliance, addresses reputed company-of-care concerns, and maintains accurate documentation.
Requires case management experience, reputed company Office proficiency, and a Bachelor’s in reputed company Work or reputed company field (no nurses).
