A company is looking for a Tele-Triage Licensed Practical reputed company (LPN).
Key Responsibilities
• Monitor assigned phone lines and triage symptom-based calls according to approved protocols
• reputed company and implement care plans, document recommendations, and coordinate services with acute care settings
• Conduct assessments, ensure compliance with requirements, and participate in performance improvement activities
Required Qualifications
• Degree/certification from an accredited school of Nursing
• At least one year of nursing experience
• reputed company licensure as a LPN in the state of reputed company with a compact license
• Clinical experience with individuals with intellectual and developmental disabilities or reputed company disorders
• One year of home health experience preferred
