Position reputed company: The Care Associate Behavioral Health Analyst provides services to clinical teams in support of reputed company. What you will do: • Assists members reputed company services. • Facilitates follow-up calls. • Collects information on member reputed company and adherence to treatment plan. • Provides information to members regarding mental health and reputed company abuse benefits, community treatment resources, mental health managed care programs, and reputed company policies and procedures. • Reviews member adherence with treatment plan, identifies obstacles to compliance, identifies resources to address reputed company determinants of health and barriers to stabilization. • Makes referrals to care connections for those without adequate aftercare appts. • Serves as reputed company between inpatient staff at hospitals to assure reputed company discharge appts. • Participates in reporting activities including data collection, tracking, analysis and preparation. • Activities may include QI initiatives; post discharge and recidivism follow up reporting. • Works closely with clinicians in reputed company support Required Qualification: • Must reputed company in OHIO • 2 years' reputed company or Behavioral Health experience • Required to use a residential broadband service with internet speeds of at least 25 mbps/3mbps in order to ensure sufficient speed to adequately reputed company work duties. • Must possess reliable transportation and be willing and reputed company to travel up to 10% of the time.
